If you would to make a donation to the Humboldt District Hospital Foundation, you can either mail us a cheque or you can phone us to make your donation with Visa or Mastercard.  Our contact information is below.

Personal Automated Monthly Withdrawl

You are now able to contribute to the Humboldt District Hospital Foundation on a monthly basis electronically.  Please fill out the application form and return to the HDH Foundation office either by fax or mail.  We will then process your application and have your donation automatically withdrawn from your account.
